The Licensed Practical Nurse provides direct patient care, administers medications, and supervises support staff, under the supervision o! f a Registered Nurse in a facility and/or the Home Health Director/Supervisor in the field. He/She is responsible for the nursing care of clients and ensures that the client Plan of Care is followed. This includes, but is not limited to providing nursing services, assisting in carrying out physician’s orders, assessing residents to note any changes in resident’s conditions, responding and reporting to resident’s changes in condition, distribution/charting of medications, distribution/count of narcotics, and responding to emergencies.

The ideal candidate for this position must be a graduate of an accredited practical nurse training program, have a current license as a Licensed Practical Nurse in this state, must pass the Care Advantage pharmacology exam, and be able to demonstrate satisfactory performance administering medications under supervision. In addition, he/she must maintain current CPR certification, have a minimum of one year experience in a health c! are setting, and must possess a current, unrestricted driver's! license in this state.

The following is a representative list of duties and responsibilities associated with this position:
-- Assists in developing, planning, implementing, and evaluating the client Nursing Care Plan, including goals and nursing actions, under the supervision of the RN
-- Provides direct Patient care based on the policies and procedures of Care Advantage/Facility and under the supervision of an RN
-- Assists the client with activities of daily living, including teaching self-care techniques
-- Reports patient responses to RN
-- Ensures the patient and family participate in the formulation of the client Nursing Care Plan
-- Reports educational needs to the RN
-- Demonstrates knowledge and skill in the administration of medications (with the exception of IV push medications)
-- Organizes assignment in a timely and efficient manner, seeks clarification from RN before implementing
-- Demonstrates an understanding o! f own limitations and seeks guidance and assistance when necessary
-- Demonstrates skill in oral and written communication, as well as interpersonal relationships with clients, families, and co-workers
-- Observes and reports symptoms and conditions
-- Documents progress notes accurately and completely
-- Assists the physician/RN in performing specialized procedures, including the preparation of equipment and supplies for treatments that require adherence to sterilization techniques
-- Shares knowledge and lends assistance to others when needed
-- Maintains confidentiality regarding patient information
-- Participates in training and in-services provided by Care Advantage and others
-- Maintains current CPR certification
