Flexible Part Time / Full Time SERVICE FACILITATOR position available in the Halifax (surrounding area), VA.

A Medicaid Waiver Service Facilitator is responsible for monitoring the services provided to individuals on the EDCD and ID/DD waivers to ensure their safety, satisfac tion, health and welfare while living independently with supports in the community.Service Facilitators meet monthly with recipients/caregivers in their home and must have reliable transportation, a strong work ethic, excellent written and verbal communication skills, and demonstrate patience and compassion. Qualified candidates will be compensated per visit and are responsible for obtaining, securing, uploading, and keeping current all necessary documentation. Strong computer skills are a must!

To be considered for this position, applicants shall possess, at a minimum, either an Associate’s degree from an accredited college in a health or human services field, or be a registered nurse currently licensed to practice in the Commonwealth and possess a minimum of two years of satisfactory direct care experience supporting individuals with disabilities or older adults; or a Bachelor’s degree in a non-health or human services field and possess a minimum of three years of satisfactory direct care experience supporting individuals with disabilities or older adults. Individuals with a Bachelor’s degree from an accredited college in a health and human service field and possess a minimum of two years of satisfactory direct care experience supporting individuals with disabilities or older adults.
