Further informations about this vacancy opportunity kindly read the description below. Evolent Health is looking for a Clinical Pharmacy Specialist to join its population health team. As a member of a team of nurses, social workers, physicians, pharmacists, health economists, and program coordinators, you will have the opportunity to make a profound impact on the lives of people living with multiple chronic illnesses. You will connect with your patients in person, on the phone and through email, in the hospital, physician office, and patient’s home - essentially however and wherever the patient needs your assistance to improve their health, bette! r understand their illness and coordinate their care.

! Responsibilities:
Patient Care

Assess the drug therapy needs of patients & monitor progress of therapy through consultations, patient education and clinical laboratory monitoring. Assess patients for medication adherence barriers and develop a plan to maximize patient’s medication success through improving or maintaining adherence to medications

Based on this assessment and in conjunction with the patient, the patient’s physician and other members of the population health team, create a care plan that will address the identified needs, remove barriers and improve the health of the patient.

Provide patients with a personal medication record that can be shared with caregivers, with prescribers, and across care transitions

Provider Support

Develop and deliver physician drug education plans & strategies (including pharmacy-specific benchmark reports) to promote cost-effective prescribing

Serve as expert in cl! inical pharmacokinetics & pharmacodynamics and is knowledgeable in drug information including contraindications and dose adjustments

Care Management Collaboration

Serve as an accessible authority on drug usage, interactions, over-dosages, compliance and other pharmaceutical information including non-formulary and restricted drug determinations

Develop and deliver pharmaceutical education to population health team

Assist in development and execution of pharmacy-specific population health strategic and tactical plans including HEDIS measure and CMS Star rating improvement

Function as liaison and subject matter expert related to client PBM systems and processes most specifically related to drug utilization management, drug prior authorization and pharmacy claim adjudication

Pharmacy Program Support

Assist in development and lead the execution of clinical pharmacy programs & initiatives and other Pharmacy and Therap! eutics activities

May serve as preceptor for pharmacy students! and pharmacy residents in accordance with established protocols

Qualifications:
Required (Specialist I)

Pharmacy doctorate degree

Completion of 1 year residency related to practice area or 3 years of equivalent clinical practice experience

Current active pharmacy license in good standing in at least one state

Required (Specialist II)

Pharmacy doctorate degree

3-5 years clinical practice experience

Current active pharmacy license in good standing in at least one state

Preferred

Board certification (either Pharmacotherapy (US-BCPS) or Ambulatory care (US-BCACP)- A plus

Certified patient educator (e.g., Diabetes, Asthma)- preferred

Health plan or PBM clinical pharmacy experience (1-2 years)

Exceptional interpersonal, presentation, oral and written communication skills

Committed to team-oriented work environment

Strong work ethic bui! lt on a foundation of proactivity

Committed to the practice of inquiry and listening

Navigate ambiguity with structured problem-solving and decision-making

Analytic with the ability to transform data & trends into actionable insight

Exceptional communication skills, both written and oral, ability to positively influence others with respect and compassion; fluency in a foreign language is a plus.

Ability to navigate ambiguity with the aid of structured problem solving techniques

Flexible work schedule â€" may need to work evenings/ occasional weekends to provide patient access and/ or follow up.

Computer proficiency in Microsoft Office
