Delivery and set up of telemonitoring technology in patient homes
Training patients on use of home telemonitoring technology and reinforcing compliance with C3-related discharge instructions
Comparison of patient medications in the home with discharge prescriptions
Documentation of primary care physician information and follow-up appointments
Pickup and refurbishme! nt of telemonitoring equipment from patient homes after monito! ring periods are completed
Occasional overnight travel may be required
Requirements
Licensed Practical Nurse (LPN), Emergency Medical Technician (EMT), Certified Nurse Aide (CNA), or Medication Technician
Minimum three years experience required (hospital and/or home health experience preferred)
Excellent patient skills and ability to maintain thorough summaries of patient interactions
Ability to quickly understand and explain use of home telemonitoring technology hardware and software
Excellent problem solving skills, especially ability to effectively evaluate patient home environment as it relates to C3 technology needs
Effective communication skills and personal etiquette, building trust with patients while understanding their conditions
Demonstrated ability to effectively communicate by phone and in person
Basic computing skills
Valid Virginia driver’s license and safe dr! iving record a must
Reliable automobile and active automobile insurance policy
American Heart Association Health Care Provider BLS certification required
Compensation/Other
Salary based on experience, with salary/bonus up to $39,000.
