The Home Health Director supervises all Home Care patients, including Private Pay, Medicaid, and Third-Party reimbursed. Reporting to the Office Administrator, the Vice President of Operations, the Administrator of Clinical Services, and the President, he or she acts as a liaison between Discharge Planners, Social Workers, Insurance Companies, Case Managers, and Staff and supervises the care of all temporary nursing employees on home care and private duty cases and all Home Health Registered Nurses (RNs) and Licensed Practical Nu! rses (LPNs).

The ideal candidate for this position is! a graduate of an accredited school of Nursing, holds a current, unencumbered license as a Registered Nurse in this state, and has two to five years of nursing experience, preferably in acute care, ambulatory care, or home health. Fluent English speaking and written communication skills, empathy and the ability to listen, good time management skills, and professional dress, speech, and behavior are all required for this position.

The following is a representative list of duties and responsibilities associated with this position:
-- Calls on Social Workers and Physicians and provides information regarding company services, including brochures and rates of service
-- Maintains relationships with clients by written and verbal communication
-- Makes initial contact with the patient and explains the services offered
-- Ensures the initial assessment of the patient is completed by an RN and that the individual performing the assessment designs a plan of c! are for the patient and places a copy in the Start of Care folder
-- Reports progress of the patient's care to the Social Workers who referred the case, if needed
-- Completes (or delegates appropriately) the Start of Care for Medicaid referrals
-- Performs rounds on patients based on the guidelines for supervisory visits
-- Helps organize monthly in-services and OSHA standards
-- Helps recruit, interview, hire, and terminate Home Health Supervisors and Nurses as required
-- Initiates the plan of treatment and re-evaluates the plan on a quarterly basis
-- Initiates preventative and rehabilitative measures, delivering or supervising skilled nursing services as needed
-- Ensures proper documentation is completed for home care and private duty cases
-- Assigns in-home supervision for nursing services provided
-- Ensures any change in a patient's mental or medical condition is reported to a physician
-- Oversees the program to instruc! t household members in the nursing and personal care of the patient -- Follows guidelines for supervisory visits
-- Provides input into the orientation, training, and supervision of the home care and private duty personnel
-- Serves as a member of the Quality Improvement Committee
-- Assists in marketing and recruitment activities, including job and vendor fairs and presentations at community activities
-- Researches and seeks out new clients and maintains relationships with current clients
-- Obtains Guarantor Forms from new patients and sends to the Billing Department
-- Calls and verifies insurance coverage, when needed
-- Obtains information required by insurance companies for billing purposes, including the physician's letter of necessity or plan of treatment, the Assignment of Benefits form from the patient, RN visit assessments as needed, and any other information pertaining to nursing required by the insurance company
-- Participates in in-service and training programs (internal or external)
-- M! aintains current CPR Certification according to the American Heart Association or Red Cross Standards
-- Maintains current Health Record annually
-- Follows the company dress code
-- Strives to develop and maintain team spirit and shares information and helps others when needed
-- Maintains confidentiality regarding patient information
-- Exhibits outstanding attendance and punctuality and takes corrective action to prevent recurring absences
-- Organizes assignments and completes them in a timely and efficient manner
-- Seeks clarification regarding assignments from the Office Administrator, the Vice President of Operations, the Administrator of Clinical Services when needed
-- Responds to unpredicted or emergency situations and reports them to the Office Administrator, the Vice President of Operations, the Administrator of Clinical Services
-- Reports concerns to the Office Administrator, the Vice President of Operations, the Administra! tor of Clinical Services when needed
-- Extends courtesy and an att! itude of cooperation when communicating with others
-- Conducts or supervises the orientation and training of employees assigned to the nursing service
-- Assigns RNs, LPNs, Certified Nursing Assistants (CNAs) or Personal Care Aids, or other nursing support staff to patients
-- Completes mock Medicaid surveys and ensures that all documents and guidelines are fulfilled
-- Understands all Medicaid and State Licensure guidelines
-- Responsible for State Licensure and Medicaid inspections and reviews
-- Completes sporadic reviews with personnel and patient files to ensure compliance
-- Assists with OSHA training for employees
-- Directs or supervises Home Health Nurses in the training of employees, as needed
-- Ensures that the company is practicing within applicable OSHA standards
-- Completes appropriate forms for the Office Administrator, the Vice President of Operations, the Administrator of Clinical Services, and the President, inclu! ding monthly statistics
