Description: Carilion Clinic is at present recruited HOME HEALTH NURSE - HHSKILLED NURSING right now, this career will be placed in Virginia. Detailed specification about this career opportunity please give attention to these descriptions. ***Flex time RN who will cover visits in the Bedford and Roanoke areas as needed.*** ***Must work a minimum of one (1) weekend per month with availability throughout the week to help with critical staffing needs.*** Must possess skills to serve as a liaison between physicians' offices and Home Care Services to coordinate necessary follow-up home care. Will assess referrals for appropriateness ensuring admission criteria are met. Will formulate and coordinate the plan of care to meet the needs of the patient. Minimum Qualifications Required: Education: Graduate of a school of nursing of an accredited college.

Experience: One (1)year experience as a Registered Nurse.Home Health experience preferred. Licensure/Certification/Registration: Must be licensed Registered Nurse in theState ofVirginia. Must have a valid Virginia driver's license. OtherMinimum Qualifications: Good verbal,written and verbalcommunication skills with diverse patient populationandexcellent assessmentskills essential.
