Summary: (Summarize primary purpose of job function) In cooperation with the Medical Director, the PACE Quality Assurance Coordinator develops, implements and evaluates all activities of the Quality Assurance and Performance Improvement (QAPI) Program. Activities relate to individuals, organizations and facilities that provide service directly or under contract to PACE. The Quality Assurance Coordinator will administer the clinical compliance program by implementing systems and processes designed to ensure compliance with applicable laws, regulations and standards. Essential Responsibilities: 1.
Maintains confidentiality of all records, medical records and information that pertain to the QAPI Program and serves as the HIPAA privacy officer. Prepares all Medical records for IDT review, QAPI Committee and any Advisory Committees. 2. Designs, organizes and implements the Compliance Program.
Monitors and measures compliance issues, including conflict of interest. Receives and documents reports of conflict of interest and non-compliance. Ensures all contractors for PACE are fully licensed and compliant. Participates on the PACE Management Team and serves as the corporate compliance officer. Coordinates with members of the Interdisciplinary Team regarding quality of care and service delivery 3.
Services as a liaison among individual program disciplines, contracted hospitals and community service providers on issues related to quality of care 4. Oversees the annual QAPI Plan update and works closely with the PACE Medical Director, Executive Director and staff of the Interdisciplinary Team to identify standards, develop indictors, collect and analyze data and provide corrective action to improve performance 10. Reviews requests for protected health information and ensures all requests for participant information have been approved by the participant through the use of the Authorization for Use and Disclosure of Health Information 12. Develops and monitors policies and procedures for quality oversight of contract services and provides related documents and information to contractors.
Works closely with the Executive Director and Program Director to receive, document and resolve contractor incidents 13. Coordinates quality assurance activities related to new enrollees and voluntary and involuntary disenrollment. Works closely with the Executive Director and Program Director to receive, document and resolve reports of participantsâ rights violations, grievances, appeals and other issues related to services and quality of care. Monitors the grievance and appeal logbooks to ensure that they are up-to-date and that the grievance and appeal process is proceeding according to standard 14.
Aggregates and analyzes information on indicators of participant risk and the care process as part of the PACE QAPI Program and provides monthly reports to the Executive Director and quarterly reports to the Management Team and the QAPI Committee 15. Conducts Participant Satisfaction Surveys every six months, aggregates and analyzes the results as part of the PACE QAPI Program and provides reports to the PACE Management Team, QAPI Committee and Participant Advisory Committee 16. Conducts on-site review of contract organizations with delegated quality assurance programs, including medical groups, hospitals and skilled nursing facilities 17. Oversees Infection and Exposure Control.
Draws upon the training, health education and disease prevention program of PACE to educate participants and staff. Oversees the procedure for infectious and bio-hazardous waste disposal to comply with PACE standards. Develops policies and procedures and a manual on Infection and Exposure Control and updates it at least annually. 18. Educates and assists PACE physicians and staff regarding QAPI Program initiatives and collects aggregate information on medical specialists care of participants that is provided to the PACE Medical Director and Medical Advisory Committee at least annually 19.
Complies with safety policies and procedures, identifying and immediately reporting any potential or actual unsafe acts or conditions to the Site Manager. Takes necessary measures to ensure a safe environment for oneself, co-workers, contractors, participants, visitors and others. 20. Performs other duties as may be required or assigned.
Minimum Qualifications: Degree and diplomas : Bachelor of Science in Nursing, Public Health, Quality Management, Health Care Management or related field. Experience : Minimum of two years in the quality assessment and performance improvement area. Professional experience administrating, aggregating and analyzing data and survey results. Licensure: Valid Virginia Drivers License with good driving record.
Verification of completion of CPR (BLS) and first aid training. Key Competencies: (Knowledge/Skills/Abilities) Ability to work effectively in a team environment as well as independently. Demonstrates sound written and verbal communication skills to convey information effectively and persuasively. Ability to facilitate and support change.
Age specific competency in working with the elderly. Outstanding organization and analytic skills including data collection, survey administration and reporting. Good interpersonal skills and ability to work with professionals at all levels of the organization. Outstanding report writing skills. Computer and software competence required.
