Organization Overview
The Hope, Unity and Freedom Center is a day operated facility that supports the! needs of adults who demonstrate a need for psychosocial day support services due to existing mental, behavioral, and/or emotional illnesses that prohibit or impede the adult ability to live independently. The Hope, Unity and Freedom Center is seeking a Clinical Coordinator for the newly opened Norfolk, VA facility.
The Hope, Unity and Freedom Center is a diverse team of professionals providing psychosocial day support to male and female adults with a primary mental health diagnosis, dual diagnosis, and/or a coexisting disorder; such as substance abuse. All social workers are qualified mental health & mental retardation professionals (QMRP/QMRHP).
With the addition of our Norfolk location, we will have three locations in Central Virginia and two location in the Tidewater region. With our multiple facilities, The HUF Center has been able to reach out to many adults who demonstrate a need for psychosocial day support services.
Our staff includes lic! ensed clinical social workers with a combined total of over 20! years of experience. This includes program nurses, management, and staff. The HUF Center is licensed by the Commonwealth of Virginia and the Department of Mental Health Mental Retardation Substance Abuse Services.
Our work helps to decrease psychiatric hospitalization and provide stabilization of individuals with less intervention in the community. By providing daily routine and structure in a safe environment each individual is able to achieve his/her best and improve his/her quality of life.

Qualifications
Must have graduated from an accredited college or university and possess a Master’s degree in Social Work or related field. Must possess a current state approved license (LCSW or LPC), must have a current CPR/First Aid, trained in Handle with Care Behavioral Management within 30 days of employment, and have an annual TB screening test. Must re! ceive satisfactory results from a criminal background check and central registry check.
Duties:
Responsible for conducting initial interviews for all potential admissions.
Responsible for facilitating assessments to ascertain service suitability.
Responsible for facilitating all facets of admissions to Psychosocial Rehabilitation.
Responsible for the process of developing individualized service plans.
Responsible for obtaining all required and supportive documentation to effectively evaluate and complete the admissions process.
Responsible for making sure the completed client file is passed to Program Manager for case management assignment.
Responsible for completing final assessment and service plans.
Responsible for duties as assigned by the Director of Clinical Services or the Executive Director.
Responsible for supervising all volunteers seeking licensure or certification.
