FullTime/Days

The Clinical Documentation Specialist (CDS) is responsible for concurrent review of inpatient medical records for p! roper physician documentation. Reviews include new admissions to the facility as well as follow-up reviews every 2 to 3 days until the patient is discharged. Facilitation of modifications to clinical documentation through concurrent interaction with physicians and other members of the health care team to ensure that appropriate clinical severity of illness is captured for the level of service rendered to all inpatients. Responsible for communicating to physicians and members of the health care team to ensure timely and accurate documentation which reflects clinical treatment, decisions, and diagnoses for inpatients.

The CDS provides education and training as needed with medical staff. Utilizes coding and clinical expertise to identify opportunities and ensure the accuracy and completeness of clinical documentation used for measuring and reporting physician and hospital outcomes.

REQ! UIRED: Graduate from a recognized School of Professional N! ursing with a valid and current Virginia Registered Nurse License and Associates or Bachelor's degree in Nursing. Knowledge of MS-DRGs and ICD-9 coding preferred.

A minimum of 5 years recent clinical nursing experience, preferably on a medical/surgical unit or minimum of 5 years recent coding experience in an acute care hospital required. Alternate 4 to 5 years experience as a Case Manager can be substituted for recent in patient experience. Advanced clinical expertise and extensive knowledge of complex disease processes with a broad clinical experience in an inpatient setting is strongly preferred.
