Job Summary:
The Care Transition Coordinator is primarily responsible for facilitating the transfer of patients! from the hospital setting to the care of a LHC Group agency ONLY AFTER the patient's physician has ordered home care services and after the patient has chosen an LHC agency as his/her provider. The Care Transition Coordinator also ensures that the agency can meet the needs of the patient after the referral is received.
Job Functions:
Visits and evaluates the patient in the hospital to obtain necessary information to facilitate the transfer.
Evaluates the patient for appropriateness for home care and obtain payer information.
Explains home care services and agency procedures to the patient and his/her family members.
Involves the family/caregivers in the educational process and assesses post-discharge educational/coaching needs.
Assesses patient's risk for readmission and develops an interim plan for patient to seek consultation or follow-up care during the transition period.
Requirements Experience Requiremen! ts
Must have one year home health experience or one year o! f hospital case management experience.
License Requirements
Must be a Registered Nurse (RN)
Independent means of transportation including valid driver's license and auto insurance.
Skill Requirements
Excellent verbal and written communication skills.
Excellent organizational skills. * Must have thorough understanding of home health qualifying criteria and coverage guidelines.
Proficient computer skills.
