We are seeking a highly motivated and energetic Wound Care Nurse to join our growing healthcare team. Our ideal candidate is an individual who is currently in a good standing with the Virginia Department of Health Professions, and the ideal candidate should demonstrate excellent clinical, customer service and leadership skills.

Job Responsibilities:
As a Wound Care Nurse, you will be responsible reviewing all resident admission assessments to assure skin risk assessments have been completed and appropriate interventions implemented. We would like them to complete body assessments on each resident every week, implementing any! new preventative interventions or treatment interventions as ! indicated by the assessment. The Wound Care Nurse should also be able to work with Certified Nursing Assistants to ensure Body Audit forms are completed as baths and showers are done and that appropriate nursing follow up has been implemented.

Other responsibilities for this position include:
Reviewing Care Tracker reports for new skin areas and assuring that all areas have been assessed by nursing staff and that appropriate prevention or treatment interventions have been implemented
Measuring all wounds weekly and reporting changes to nursing staff
Initiating and performing wound care following facility protocols and physician orders
Completing accurate and timely documentation of wound and skin assessments and treatments according to facility policies and nursing procedures
Communicating regularly with physicians to update on wound and skin conditions
Completing physician order forms for any new orders received
Assuring care pla! ns are implemented and updated timely to accurately reflect the resident’s condition and needs
Working closely with MDS Coordinator to assure supporting documentation is available for accurate coding
Conducting weekly Wound Meeting, bringing to the group appropriate information to oversee wound care and prevention program for facility
Participating in the weekly Risk Meeting to assure appropriate interventions and/or treatments for any resident identified at risk for skin issues
