Summary: As an expert clinician, is responsible for the coordination of all services/systems to ensure an organized, multidisciplinary approach to the delivery of care to all adult Hematology/Oncology patients. Provides expert care incorporating principles and theories of nursing and related fields to achieve identified patient outcomes. Assists the Director/Manager in working with the staff to ensure that the plan is carried out appropriately and consistently. Acts as a liaison to physicians and other internal / external departments or providers to achieve the goals of the department/program/ institution. Essential Duties and Responsibilities 1.0 Assures the delivery of comprehensive cancer services through the coordination of and direct or indirect provision of expert patient care for the adult hematology/ oncology population 1.1 Coordinates and monitors all phases of cancer services including: prevention/screening, diagnosis, treatment, survivorship, death and dying for patients and caregivers. 1.2 Maintains a retrievable patient list that includes key information such as but not limited to name, DOB, patient status, treatment plan status 1.3 Acts as a liaison between patients, physicians and the hospital in transitioning the patient throughout the health care system including inpatient and outpatient environments 1.4 Collaborates with financial counselors and social services to assure completion of financial assessments 1.5 Coordinates with appropriate departments / agencies verification of third party pay benefits making appropriate referrals for those patients with no/limited health care benefits 1.6 Assists with completion of health care finance paperwork (medical necessity letters, appeal of denials, prior authorization) 1.7 Collaborates with Referral Coordinator to coordinate placement of new patients with correct service and within defined department time frames 1.8 Coordinates plan of care with assigned oncology team/service 1.9 Collaborates with Social Services, CCC/Outcomes Manager, and home health agencies regarding outlined plan of care; assists with referrals, problem solving or overcoming obstacles to enable patients to achieve optimal level of functioning. 1.10 Routinely monitors service schedules. Adjusts appropriately to maximize effective, efficient patient flow through intradepartmental (MBRCC) lab, clinic or infusion center 1.11 Facilitates /coordinates scheduling of interdepartmental and external agency services (i.e. POC, Radiology, Eye Center, etc.) related to the plan of care. 1.12 Attends inpatient /discharge rounds, tumor boards, etc as appropriate to obtain necessary information for communication with other departments/agencies regarding patient status and health care needs 1.13 Assures effective communication and collaboration among the health care team members, addresses inter and intradepartmental issues. 1.14 Coordinates long- term follow up of patients with their local/referring physicians 1.15 Maintains written/verbal communication with referring institutions and physicians 1.16 Assist with completion of health care related paperwork (i.e. Leave of absence, FMLA, return to work/school, etc) 2.0 Provides coordinated and comprehensive patient care including assessment, care planning for expected outcomes, nursing interventions and evaluation using a multidisciplinary team approach 2.1 Applies nursing process to each patient and/or family based on experience, theoretical knowledge and clinical expertise 2.2 Bases decisions made/actions taken on relevant/current scientific principles, established standards of care and/or research findings. 2.3 Performs an education needs assessment on all new patients. 2.4 Provides initial teaching to patients and caregivers Performs ongoing teaching to complex/ high-risk patients and caregivers Delegates and coordinates teaching of intermediate or low risk patients and caregivers as appropriate. 2.5 Evaluates the effectiveness of teaching efforts and modifies education plan as needed. 2.6 Develops service specific patient / caregiver education materials 2.7 Completes initial nursing assessment on all new patients Develops or coordinates development of individualized plans of care based on psychosocial needs, medical plan of care and health beliefs of the patient and caregivers. 2.8 Assists medical team with on-going treatment planning 2.9 Develops a caring, compassionate relationship with the patient and/or caregivers 2.10 Demonstrates expertise in caring for patients across the age continuum 2.11 Coordinates and or participates in appropriate response to a life-threatening emergency. 2.12 Provides hands-on patient care in the lab, clinic or infusion center by performing nursing assessments, chaperoning, assisting with or performing procedures, phlebotomy, IV therapy and other direct care as appropriate to keep patient flowing through service areas Triages incoming patient telephone calls Performs proactive outgoing telephone triage by routinely contacting high-risk patients and caregivers to check on health status and service needs 3.0 Uses interpersonal and problem-solving skills to manage patient care/ services in complex situations by mobilizing organizational and community resources 3.1 Exhibits positive professional behaviors on a consistent basis 3.2 Communicates effectively with physicians, nurses, and other personnel in problem identification and resolution, taking appropriate action to resolve issues 3.3 Completes projects and other duties on time and as assigned 3.4 Promotes a continuous improvement philosophy for the identification and resolution of issues as a mechanism to enhance professional practice 3.5 Consistently answers pagers and telephone calls in a timely manner 4.0 Demonstrates accountability for nursing practice and identifies own learning needs. 4.1 Actively participates in professional nursing organizations related to specialty. 4.2 Demonstrates self-motivated professional and personal development by assuming responsibility for continued learning.= Seeks learning opportunities both internally and externally, applies knowledge gained to patient care delivery and shares with peers to positively impact practice. 4.3 Maintains chemotherapy certification 4.4 Supports and participates in hospital wide and unit based shared governance teams, education, in collaboration with the Director/Manager, designs, develops, implements, and evaluates the patient and staff education programs. 4.5 Completes annual self-evaluation to validate adequate job knowledge/skills; Creates an annual individual development plan based on this evaluation and submits it to Director/Manager 4.6 Provides professional and/or public in-services 4.7 Participates in community service activities annually 4.8 Identifies knowledge deficits and/or performance discrepancies of staff and reports to the Director/Manager including corrective action steps taken or suggested plan of action to correct deficit/ discrepancy 4.9 Participates in/ leads departmental and or patient care meetings as assigned. 5.0 Assists the Director/Manager and /or multidisciplinary team in quality improvement activities 5.1 Bases decisions made/actions taken on relevant scientific principles, established standards of care and/or research findings 5.2 Identifies, defines and monitors measurable clinical indicators annually to maximize patient outcomes 5.3 Collects, analyzes and presents data related to quality improvement activities and assists in developing, and implementing corrective action plans, annually. 5.4 Integrates information gained through quality improvement activities and clinical nursing research into practices. 5.5 Participates in clinical research activities Coordinates patient care needs with CTRU 5.6 Develops/revises/follows established standards of care, standards of practice and policies and procedures. 6.0 Participates in Performance Improvement activities as appropriate 6.1 Identifies knowledge deficits of assigned staff and provides corrective action, and if appropriate, reports discrepancy and follow- up to the Director, or designee, regarding the need for individual or unit -wide development plans. 6.2 Recognizes performance discrepancies of staff and takes corrective action and if appropriate, reports discrepancy and follow- up to the Director, or designee 6.3 Provides feedback as requested for evaluating employee performance 6.4 Assists in the orientation of new employees 6.5 Initiates action to manage complex system or unit issues and seeks assistance from the Director/Manager appropriately. 7.0 Other duties/projects as assigned Performance Standard: Adheres to the established Performance Expectations for WVUH Employees in the areas of People, Service, Performance Improvement, Shared Values & Culture. Position Requirements

  • BSN required.* MSN preferred • Three to five years experience with relevant clinical experience required, hematology/oncology experience preferred. • Chemotherapy certification required within 3-6 months of hire • OCN certification, preferred at time of hire, required within two years of hire • Current Licensure to practice professional nursing in West Virginia * Above qualification may be waived in lieu of significant relevant experience or accomplishments.
